is there any warranty on this ? I used it once and broke off in bolt the first thing, like it was made out off glass!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

CAVEAT EMPTOR. SpeedOut does ***NOT*** honor their lifetime warranty when it is not ordered directly from their website, and instead requires you to contact the Amazon.com seller for a replacement. (The Amazon seller sent me a new SpeedOut for free, at their own loss-and to their credit-without me shipping mine back to them.) Will this work on #4 machine screw with no head?

yes- you do have to drill into the middle of your stripped screw - the use your speedout with a pair of pliers or a rachet handle

Question 3

I have a stripped screw down inside a metal hole about an inch.  Will this get that out?  No idea of the size.

Maybe, but only if both sets of threads are not damaged and you can use a drill press with a vise. With damaged threads you may not be able to penetrate deeply enough into the stripped screw to be effective, which also maybe true with good threads if you are on the far side of an inch.
